Insurances Accepted and Billing Policies

Please be prepared to pay your co-pay, co-insurance and/or deductible (if applicable) at the time of your appointment. We accept credit cards and debit cards, in addition to health savings account cards.

Damaris Aragon accepts many major health insurances. Please see below for a current list of the insurances I accept, and my billing policies.

Billing Information

Your co-pay, co-insurance and deductible (when applicable) are due at your appointment time. Any balances over 30 days will incur a $5.00 billing fee per appointment, per cycle.

Please check with your insurance company to find out your benefits and responsibilities, including if you have a deductible, and the amount of your co-pay.

Please obtain a referral from your primary care provider if this is required by your insurance provider. Please double check with your insurance company that I am an in network provider with your insurance company. Please bring your driver’s license and insurance cards with you to every appointment.

Please note you are responsible for updating your billing information, including changes to your address and insurance information in the event this information should change.

Balances past due for 90 days will be sent to an outside collections agency.

Fees for Medical Records

Please go to the WAC 246-08-400 Statute page for details regarding allowed fees for copying or transferring medical records. These fees apply if you want a copy of the medical records for yourself. These fees also apply if these medical records are required in regards to a disability claim, and other applicable situations. No fees are charged for medical record transfer to other care providers, as needed for continuity of care.
