Medication Management with No Waiting List

To prepare for an initial appointment, please follow these instructions:

Step 1

Fill out your demographic sheet.

New Patient Intake Form

Check for a confirmation email of receipt of your form.

Step 2

The link for your patient portal is MYIO Link.
Check for invitation to MYIO (Your Patient Portal), where the forms necessary for your appointment will be located. Your invitation will come via text or email. The invitation will include a code for you to register for MYIO. Set this up within 24 hours of receipt.

Step 3

Fill out the forms required for your appointment.

These forms include your medical and medication history, and screening forms. Once these are filled out, you are ready for an appointment.

Step 4

Message us through MYIO to let us know you are ready, and we will schedule you.

If you are telemedicine, this is the platform used – Doxy.me. Telemedicine appointments are not in MYIO.

Documents Needed

The following ID’s and documents are needed at your first appointment with me:

  • Driver’s License.
  • All Insurance Cards.

Disability Policy

Damaris Aragon does not fill out any paperwork related to permanent disability. If you want to apply for permanent disability, I recommend contacting the social security administration, or other appropriate government agencies. These agencies will set you up with an independent medical examiner, who will then do the evaluation. For any of my patient’s applying for disability, patients can sign a release form to release their medical records to the disability department. There will be a charge for this as defined by law. Please see the insurance page for details.

Aragon Mental Health, LLC does not provide court evaluations, and is not a forensic psychiatry office. Forensic (court) work and forensic psychiatry is a specialty that we are not trained in. The court system can provide you with information about forensic psychiatry. We do not provide legal opinions, do evaluations, or testify for disability or child custody cases.

Worker’s Compensation

We are not contracted with Labor and Industries to provide initial evaluations, or ongoing care related to worker’s compensation claims. Please contact your employer, or the Labor and Industries department, so that they can set you up with a Labor and Industries contracted provider.
